Competence Center EHRbase – Open-source data platform for the innovative e-health systems of the future

Data can improve healthcare—provided it’s made available and can be compared and interpreted at the right time, in the right place, and in the right format. However, most existing healthcare application systems are not designed to support data exchange. Vendor-specific, proprietary software systems form countless silos that prevent data from being used efficiently for the benefit of patients. 

What’s needed is a modern, open information architecture for today’s healthcare system that can be used to decouple applications from data and enable native interoperability. The solution is EHRbase, the open-source health record platform based on open specifications, information models, and interfaces.

EHR-Base

EHRbase is an open-source implementation of the openEHR specification that already allows data to be decoupled from application systems and enables native interoperability in countless national patient health records and hospital information systems.

Open source has changed many industries for the better, and now it’s the turn of healthcare. That’s why vitagroup initiated the EHRbase open-source project. EHRbase is an open-source version of the openEHR technology and encourages community involvement. 

Open platform architecture

EHRbase provides an openly specified and vendor-neutral software back end for clinical application systems and electronic health records. It’s based on the openEHR specifications, an open platform architecture for developing flexible and interoperable e-health systems. Applications based on openEHR share a common information architecture, i.e. they “speak” the same technical language. Instead of having to exchange data between application systems, there is shared access to it. This enables native interoperability between applications. Thanks to standardised and open data models, data from all kinds of sources can be compared and analysed. This is exactly what is needed for predictive models, for instance—and improved healthcare. 

Building a better future with open source

Because it’s available under an Apache 2 licence, EHRbase Core is free to use for commercial and non-commercial purposes, giving interested parties and users cost-neutral and straightforward access to the openEHR technology. You can use open data models and tools, such as the software development kit, to develop interoperable application systems in next to no time.

Comprehensive features

EHRbase offers comprehensive features for defining, storing, and querying clinical data. All data points in EHRbase are validated and versioned as well as being readable and writeable via the official openEHR REST API. The flexible database automatically and repeatedly adapts to your new clinical requirements and is also guaranteed to remain easily and securely accessible in the long term thanks to model-based technology (Archetype Query Language). All transactions are logged in a legally secure manner. 

Easy integration with FHIR®

FHIR helps to link data silos, while openEHR prevents them from being created in the first place. EHRbase offers the FHIR Bridge integration solution in order to connect the open platform ecosystem with existing system landscapes. EHRbase also enables direct connection to a FHIR terminology server so that data can be validated and retrieved based on standardised terminologies such as SNOMED CT and LOINC. 

Active community

vitagroup and the Hanover Medical School (MHH, Peter L. Reichertz Institute for Medical Informatics) are the initiators of EHRbase. As part of the HiGHmed Consortium, the two organisations are working together on the continuous development of EHRbase. As is typical of open-source projects, the brightest talent wants to be involved! EHRbase developers are visionaries because they actively work on the source code—and are therefore participating in the future of healthcare. They are either part of the core team or support vitagroup and the Hanover Medical School through regular contributions.

Most hospital data is still trapped inside proprietary databases. Communication interfaces are rare; data migration is expensive. EHRbase allows hospitals to enhance the capabilities of their monolithic electronic medical record (EMR) with agile methods by introducing a bi-modal approach: the traditional EMR is complemented by a vendor-neutral and open clinical data repository, which makes data centrally available and allows for the development of innovative, data-driven solutions.  

Open platforms free both data and applications by making them available in a portable and interoperable way from the outset. EHRbase thereby lays the groundwork for advancements like lifelong electronic health records—the primary objective of most regional and national e-health programs. Not only can data be exchanged between organisations, but it can also be actively used to improve cross-sectoral processes. EHRbase supports large-scale infrastructures and gives operators full control over data models and the procurement process.  

Software companies also benefit from greater control, especially in relation to development costs. EHRbase builds digital healthcare applications on future-proof standards and brings them to market quickly. The openEHR specification lowers development costs, as it supports even the most demanding use cases, including EMRAM Stage 6 certified hospital information systems and national e-health backbones. 

Start-ups in particular often put forward superb use cases that would not only optimise processes and improve healthcare from a theoretical standpoint, but also have to function in larger, practical contexts—integrated into programmes and healthcare models. Consistent standards, principally in relation to data models, are therefore essential. As a reliable back end and repository technology, EHRbase leaves organisations free to concentrate fully on processes and the user experience.  

Scientific and research institutions, such as university hospitals, can use EHRbase to overcome the challenge of linking and analysing large volumes of healthcare and research data in order to generate new medical knowledge—and exchange this data within research networks. EHRbase helps organisations to integrate and normalise data, provides reusable and machine-readable information models, and makes data queryable and accessible. The more research databases and clinical registries are developed on the basis of openEHR and reuse standardised data models, the more EHRbase’s value as a research platform grows.
